The form begins with the heading: "The following instructions and forms are intended to supersede all former instructions issued on the subject of Revolutionary and other Pensioners, and all Vouchers must be made out to conform substantially to these instructions, or they will be rejected at the Treasury". The form contains applications not only for former soldiers and affidavits for widows of deceased veterans, but an application for sailors and veterans of privateers.
