Fully engrossed and signed in her hand in black ink. Along with her AFTRA Health Fund identification card (not signed), noting her policy as "Group Policy #C2856", imprinted at bottom: "Ciccone, Madonna, ID # 366-70-9123", and expiring "6/30/90".
