War-date partly-printed D.S. filled-out in his hand, 2pp. folio, New York, Sept. 30, 1864, in whichGreene writes extensive answers to questions concerning his service record. In part: "...[engaged in]...Battle of Winchester under Gl. N.P Banks, 25 May, 1862, Battle of Cedar Mountain, Commanding 3d. Brigade2 Division...Battle of Antietam Comd. 2nd Div....Battle of Chancellorsville comd. 3rd Brig....Battle of Gettysburg Comd. 3rd. Brig. 3 Div. 12 A.C...Severely wounded in the face at the Battle of Wauhatchie Tenn. in a night attack by the enemy 28 Oct. ,1863, while in command...musket ball entered the left corner of the nose passing through the upper jaw, and through the right cheek....".
