[ Autograph ] Holocaust D.S., 3pp. folio, Krakau, Poland, Nov. 12, 1941, being a detailed application form which the Germans required all medical professionals to complete in order to be allowed to continue their practice, This particular form was filled out by Ruth Henstein, a 31 year old nurse. As would be expected, the questionnaire required detailed information about one's family and religious background. Unfortunately for Ruth, this information would be used against her. At the top of the application, about four inchesabove her picture, a rubber stamp has marked Ruth's application with two Stars of David and the word "Jew" in German in large letters. A sad and personal reminder of a great human catastrophe. Fine.
